|Description:||Comunilife Inc., a multi-service, non-profit organization founded in 1989, offers a continuum of residential programs for adults with HIV/AIDS and/or serious mental illness, a licensed mental health clinic and other programs in NYC. Our mission is to improve the quality of life and create a healthier tomorrow for New Yorkers with special needs in the Hispanic and broader communities. |
|Duties:||The Case Manager provides services for adults living with HIV/AIDS, who reside in our 60-bed. |
-Perform all activities related to case management for assigned caseload including initial assessment(s), timely comprehensive service plans, service plan reviews and progress notes.
-Maintain and document liaisons with other agencies providing services to assigned cases.
-Provide supportive counseling and crisis intervention as needed.
-Monitor compliance and management of the clients health, mental health and substance abuse treatment needs.
-Provide assistance with all ADL activities to maintain clients independence in the community.
-Advocacy and referral as needed.
-Monitor and re-assess clients needs for treatment and community education on chemical dependency related issues.
|Qualifications:||REQUIRED KNOWLEDGE and SKILLS:|
-Experience with HIV/AIDS afflicted individuals, chronic mental illness, substance abuse individuals and their families
-Strong interpersonal and communication (verbal and written) skills
REQUIRED EDUCATION and EXPERIENCE:
-2-3 consecutive years of work experience in substance abuse and HIV/AIDS issues.
-Valid CASAC or CASAC-T preferred
-Bilingual skills in English/Spanish preferred
