Description:Comunilife Inc., a multi-service, non-profit organization founded in 1989, offers a continuum of residential programs for adults with HIV/AIDS and/or serious mental illness, a licensed mental health clinic and other programs in NYC. Our mission is to improve the quality of life and create a healthier tomorrow for New Yorkers with special needs in the Hispanic and broader communities.

Duties:The Case Manager provides services for adults living with HIV/AIDS, who reside in our 60-bed, HASA-funded permanent congregate facility. Responsibilities include completing comprehensive assessments and all required documentation, developing service plans and conducting at least weekly face-to-face visits with residents. The Case Manager develops a supportive relationship with clients, assists them in attaining their goals, ensures that clients pay rent and other bills, and links clients to community-based medical, mental health, and substance abuse treatment programs and all needed services.
Qualifications:We are looking for a caring, supportive, organized and dependable case manager who finds it rewarding to work with clients with HIV/AIDS, many of whom have co-occurring substance abuse and mental health disorders. A team player who collaborates with clients, staff and community providers; the case manager should also possess excellent verbal, written and active listening communication skills and a flexible schedule.

Bachelorís Degree in Psychology, Social Work, Human Services, related field;
