|Description:||Comunilife has an opening at DHS regulated Safe Haven I program located in the Bronx. Safe Haven I provides transitional housing and case management services to homeless individuals. Participants are referred from DHS and street homeless outreach teams. The program secures permanent housing for participants.|
The Senior Social Worker will conduct psychiatric and other assessments, as well as develop and implement person-centered treatment plans, and supportive services.
-Assessing clients to identify interrelated issues of substance abuse, health, behavioral health, and other care needs and determine appropriate internal and external referrals and preparing psycho-socials;
-Providing individual and group counseling;
-Facilitating psycho-education groups in relapse prevention, health literacy, and navigating mental health and substance abuse systems, etc. for residents their family and/or support networks;
-Providing CCM services as identified for each client;
-Assisting clients in achieving and maintaining functional improvements in self-care, employment, or personal relationships;
-Assisting clients in achieving housing stability through discharge and post-discharge;
-Engaging in discharge planning and linking clients with at least 90 days of post discharge services.
|Qualifications:||REQUIRED EDUCATION and EXPERIENCE:|
-Masters degree in Social Work (MSW) required. New York State LMSW (License Masters in Social Work) preferred
-Two (2) years working with homeless populations, mentally ill patients, persons with chronic health conditions and/or substance abuse disorders.
-Bilingual skills in English/Spanish a plus
