Location:New York, NY
Employment Type:Full Time
Department/Program:Respite Program
Description:Comunilife's innovative  Social/Medical Respite Program is a short-term residential program for  patients who are currently hospitalized, are medically cleared for discharge but do not have suitable housing.  The program’s goal is to reduce preventable re-hospitalizations and emergency room visits by providing a safe and temporary bed for the patient to complete their recovery.  While in the Respite Program, patients are able to access medical and mental health care along with other supportive services.  The Respite team works diligently with the patient to ensure medical/mental health linkages are maintained and develop a tailored Housing Plan for the patient that will result in successful housing placement within 90 days from the patient's admission to the Respite Program.  This is a very exciting time for the Respite program and expansion is currently in development as hospitals and managed care companies see the value, cost efficiency and patient benefits of our Respite program.
